What

A 5-month nutrition incentive and education program intended
to connect participants with fresh, locally sourced produce and nutrition education.

When

From June-October, participants will receive $25 in vouchers once per month to spend on produce, dairy, meats, and other eligible items from participating vendors throughout Washington County.

Who

Eligible patients who receive Care Management services at participating Glens Falls Hospital Health Centers in Granville, Whitehall, Hudson Falls, Greenwich, Salem, and Cambridge

How

Participants must be screened and referred by a Care Manager at one of the participating sites. Please contact your Care Manager for more information about program eligibility and how to sign up.
